Do we need to see the first three movies to understand this?

You definitely need to know the basics of John Connor and Skynet. If you haven't seen The Terminator or T2: Judgment Day, this movie will feel like a random collection of explosions. You can safely skip Terminator 3: Rise of the Machines, though.
