Is Mistborn too violent for a 12-year-old?

It depends on the kid, but generally no. The violence is 'PG-13'—there are sword fights and people get hurt, but it isn't described with the lingering, graphic detail you'd find in George R.R. Martin. The 'Steel Inquisitors' are the only truly disturbing part due to their spikes-in-eyes appearance.

Is there any romance or 'spice'?

There is a central romance between Vin and Elend, but it is very wholesome. There is no explicit sexual content or 'spice' in the original trilogy. Sanderson is a practicing Mormon and his 'Cosmere' books are famously clean in that department.

Do they need to read the whole trilogy at once?
